This paper describes the unique values of, challenges within, and opportunities presented by embedded ELSI ethnography. Drawing from our six‐year embedded ELSI study of the WISDOM (Women Informed to Screen Depending on Measures of Risk) trial, we present three examples of the variable ways we engaged with the WISDOM trial's scientific team. WISDOM is a preference‐sensitive, pragmatic, randomized controlled trial of risk‐based breast cancer screening informed by genomics. Our embedded ELSI approach included multiple modes of engagement: (a) Trial investigators sought (...) bioethics expertise; for example, we were asked to consult on traditional bioethics topics such as consent as well as more complex questions about whether to implement a specific risk‐assessment approach on only one race‐defined group of trial participants. (b) As the ELSI investigators, we identified ethical concerns in issues that surfaced via ethnography and considered them in the Bioethics Working Group; for example, there were concerns about the implementation processes that the WISDOM team viewed as logistical challenges. (c) Our presence in WISDOM working group conversations and our expertise on classic social science topics, including race and gender, offered opportunities to engage on the spot with topics such as how to include transgender participants in a trial initially focused on “women's health”; through such engagement, the value of social science became clear to the trial investigators. The paper elaborates on the dynamic relationship between our ethnographic observations, the Bioethics Working Group discussions, and the contributions of each to our real‐time ELSI interventions in these examples. The methods and experiences we describe here suggest modes of engagement that, in combination, have the potential to expand the role of ELSI to offer real‐time intervention on issues related to equity, inclusion, and justice in genetic and genomic research. This article examines the common-sense and methodical ways in which “the citizen” is produced and enrolled as an active participant in “sustainable” regional planning. Using Membership Categorization Analysis, we explicate how the categorization procedures in the Foreword of a draft regional planning policy interactionally produce the identity of “the citizen” and “civic values and obligations” in relation to geographic place and institutional categories. Furthermore, we show how positioning practices establish a relationship between authors (government) and readers (citizens) where both are (...) ascribed with the same moral values and obligations toward the region. Hence, “the citizen” as an active participant in “sustainable” regional planning is viewed as a practical accomplishment that is underpinned by a normative morality associated with the task of producing orderliness in “text-in-interaction.”. a few months into the covid-19 pandemic, Marilyn Fischer and Judy Whipps were commiserating about an isolated future that seemed to stretch out with no end in sight. They came up with the idea of starting their own Scholars Circle, inspired by the November 2019 Feminist Pragmatist Colloquium in Rochester, New York. At that conference, participants could submit abstracts of work at any stage of development. Participants were grouped in small circles of three or four, with each person sharing their (...) work and giving feedback and suggestions to the others. To understand public discourse in the United States on genetic causation of behavioral disorders, we analyzed media representations of genetic research on addiction published between 1990 and 2010. We conclude first that the media simplistically represent biological bases of addiction and willpower as being mutually exclusive: behaviors are either genetically determined, or they are a choice. Second, most articles provide only cursory or no treatment of the environmental contribution. A media focus on genetics directs attention away from environmental factors. To understand better why evidence of student cheating is often ignored, a national sample of psychology instructors was sampled for their opinions. The 127 respondents overwhelmingly agreed that dealing with instances of academic dishonesty was among the most onerous aspects of their profession. Respondents cited insufficient evidence that cheating has occurred as the most frequent reason for overlooking student behavior or writing that might be dishonest. A factor analysis revealed 4 other clusters of reasons as to why cheating may be (...) ignored. Emotional reasons included stress and lack of courage. Difficult reasons included the extensive time and effort required to deal with cheating students. Fear reasons included concern about retaliation or a legal challenge. Denial reasons included beliefs that cheating students would fail anyway and that the worst offenders do not get caught. Valorising the biocultural heritage of common goods could enable peasant farmers to achieve socially and economically inclusive sustainability. Increasingly appreciated by consumers, peasant heritage products offer small farmers promising opportunities for economic, social and territorial development. Identifying the obstacles and levers of this complex, multi-scale and multi-stakeholder objective requires an integrative framework. We applied the panarchy conceptual framework to two cases of participatory research with small quinoa producers: a local fair in Chile and quinoa export production in Bolivia. In both (...) cases, the “commoning” process was crucial both to bring stakeholders together inside their communities and to gain outside recognition for their production and thus achieve social and economic inclusion. Despite the differences in scale, the local fair and the export market shared a similar marketing strategy based on short value chains promoting quality products with high identity value. In these dynamics of biocultural heritage valorisation, the panarchical approach revealed the central place as well as the vulnerability of the community territory. As a place of both anchoring and opening, the community territory is the privileged space where autonomous and consensual control over the governance of common biocultural resources can be exercised. In 2018-2019, at the Keck School of Medicine of the University of Southern California, we developed and piloted a narrative-based health systems science intervention for patients living with HIV and medical students in which medical students co-wrote patients’ life narratives for inclusion in the electronic health record. The pilot study aimed to assess the acceptability of the “life narrative protocol” from multiple stakeholder positions and characterize participants’ experiences of the clinical and pedagogical implications of the LNP. Students were recruited from (...) KSOM. Patients and staff were recruited from the Maternal, Child, and Adolescent/Adult Center for Infectious Disease and Virology at Los Angeles County+USC Medical Center. Ten patients, seventeen students, and ten MCA staff participated in the pilot study. Qualitative methods were used to gather data from students’, patients’, and staff’s perspectives. Three themes emerged from the thematic analysis: patients’ life narratives conveyed their unique life experiences and voices; the protocol could result in wide-ranging effects on HIV care; the LNP enabled students to contribute value to patients’ healthcare. Across groups, participants considered the LNP an acceptable intervention. The LNP, its limitations, and implications for HIV care, narrative medicine, and health information technology are presented. (shrink)

The purpose of this article is to provide a critical examination of two aspects of culture and biomedicine that have helped to shape the meaning and practice of genetic testing for breast cancer. These are: (1) the cultural construction of fear of breast cancer, which has been fuelled in part by (2) the predominance of a ‘risk’ paradigm in contemporary biomedicine. The increasing elaboration and delineation of risk factors and risk numbers are in part intended to help women to contend (...) with their fear of breast cancer. However, because there is no known cure or foolproof prevention for breast cancer, risk designations bring with them recommendations for vigilant surveillance strategies and screening guidelines. We argue that these in effect exacerbate women’s fears of breast cancer itself. The volatile combination of discourses of fear, risk and surveillance have significant ethical and social consequences for women’s lives and well-being. Genetic testing decisions are made within this context; if nurses understand this context they can play an important role in helping women to cope with the anxiety and fear of breast cancer risk. ABSTRACTDifficulties with inhibiting fear have been associated with the emergence of anxiety problems and poor response to cognitive–behavioural treatment. Fear inhibition problems measured using experimental paradigms involving aversive stimuli may be inappropriate for vulnerable samples and may not capture fear inhibition problems evident in everyday life. We present the Fear Inhibition Questionnaire, a self-report measure of fear inhibition abilities. We assess the FIQ’s factor structure across two cultures and how well it correlates with fear inhibition indices derived experimentally. Although ethics consultation is offered as a clinical service in most hospitals in the United States, few valid and practical tools are available to evaluate, ensure, and improve ethics consultation quality. The quality of ethics consultation is important because poor quality ethics consultation can result in ethically inappropriate outcomes for patients, other stakeholders, or the health care system. To promote accountability for the quality of ethics consultation, we developed the Ethics Consultation Quality Assessment Tool. ECQAT enables raters to assess the (...) quality of ethics consultations based on the written record. Through rigorous development and preliminary testing, we identified key elements of a quality ethics consultation, established scoring criteria, developed training guidelines, and designed a holistic assessment process. O artigo que ora apresentamos problematiza a formação e os enfrentamentos da Associação Liberdades Poéticas para atender ao Objetivo Número 16 da ONU, “Paz, Justiça e Instituições Eficazes”. Trata-se de um grupo de voluntários, composto por pessoas de diferentes sexualidades, formações acadêmicas, atuações profissionais e faixas etárias, criado em 2020 durante a pandemia, que tem como premissa reconhecer a prática da leitura como um Direito Humano, indispensável à humanização e à transformação dos sujeitos. Atuamos com remição de pena por leitura (...) no sistema prisional paulista, que confere, às pessoas privadas de liberdade, quatro dias de redução na sentença para cada obra lida, em um limite de dez títulos/ano. Nossa fundamentação teórica sustenta-se em Antonio Candido, em Lynn Hunt, no Conselho Nacional de Justiça (CNJ), na Constituição Federal, entre outros. Concluímos que, apesar da orientação geral da ONU e dos esforços do governo federal com a publicação da Resolução 391/2021, do Conselho Nacional de Justiça, para flexibilizar a remição de pena por meio da leitura, agentes do sistema penitenciário no estado de São Paulo ainda operam sob uma lógica autoritária que não corresponde à do CNJ, tampouco à ODS 16. A medical information commons is a networked data environment utilized for research and clinical applications. At three deliberations across the U.S., we engaged 75 adults in two-day facilitated discussions on the ethical and social issues inherent to sharing data with an MIC. Deliberants made recommendations regarding opt-in consent, transparent data policies, public representation on MIC governing boards, and strict data security and privacy protection. Community engagement is critical to earning the public's trust.

A novel Protocol Ethics Tool Kit (‘Ethics Tool Kit’) has been developed by a multi-stakeholder group of the Multi-Regional Clinical Trials Center of Brigham and Women9s Hospital and Harvard. The purpose of the Ethics Tool Kit is to facilitate effective recognition, consideration and deliberation of critical ethical issues in clinical trial protocols. The Ethics Tool Kit may be used by investigators and sponsors to develop a dedicated Ethics Section within a protocol to improve the consistency and transparency between clinical trial (...) protocols and research ethics committee reviews. It may also streamline ethics review and may facilitate and expedite the review process by anticipating the concerns of ethics committee reviewers. Specific attention was given to issues arising in multinational settings. With the use of this Tool Kit, researchers have the opportunity to address critical research ethics issues proactively, potentially speeding the time and easing the process to final protocol approval. A recent article in this journal described practical and conceptual difficulties faced by public health researchers studying scabies outbreaks in British residential care facilities. Their study population was elderly, decisionally incapacitated residents, many of whom lacked a legally appropriate decision-maker for healthcare decisions. The researchers reported difficulties securing Research Ethics Committee approval. As practicing healthcare ethicists working in a large Canadian research hospital, we are familiar with this challenge and welcomed the authors’ invitation to join the discussion of the ‘outstanding (...) ambiguities and further questions’ that their experience uncovered. We propose a Power of Attorney for Research as one substantive solution to help address the problems they identified. Although we acknowledge the familiar shortcomings associated with Advance Directives in the clinical context, we believe that Powers of Attorney for Research Participation, accompanied by Advance Research Directives, may increase the likelihood of gaining deeper understandings of potential participant’s values and priorities and how they might apply to foreseeable research opportunities. Los suscritos acuerdos de paz entre el Gobierno de Colombia y las FARC- EP (Fuerzas Armadas Revolucionarias de Colombia, Ejército del Pueblo) son tal vez el avance en materia de paz, más grande que ha tenido Colombia, y por qué no, Latinoamérica en los últimos tiempos. El acuerdo de paz firmado en el 2016 ha generado la posibilidad de vivir en paz a toda una Nación, con miras a favorecer la reorganización de la sociedad civil que ahora puede congregarse con (...) mayor libertad en sus territorios. Ello ha permitido darle paso a las principales víctimas, campesinos, para crear escenarios en busca de la paz como las “Tertulias campesinas por la paz” en una marco de pedagogía que procura generar en sus despliegues por el territorio acciones de resistencia pacífica y conciliatoria. En esta investigación fue determinante reconocer el papel de las expresiones, estrategias y narrativas esperanzadoras que acompañan la implementación de acciones con sentidos pedagógicos. “Las tertulias campesinas por la paz” le dan lugar a una infinidad de estrategias culturales, pedagógicas y relacionales que posibilitan una apuesta rural elaborada desde una visión ontológica que permite su funcionamiento y abre paso a una estrategia de recomposición del tejido social en la zona más afectada por las dinámicas de violencia movilizadas por el conflicto armado interno en Colombia. I react to their comments on my 2015 book Potentiality: From Dispositions to Modality, and in doing so expands on some of the arguments and ideas of the book.

Dos filósofos dialogan sobre cómo definir en nuestros días la barbarie desde la filosofía política actual. Barajan para ello tres tipos de respuestas. La respuesta ilustrada es la que considera que la diversidad de concepciones del bien de nuestras sociedades es un hecho pernicioso para el desarrollo de la Humanidad, y que hay que imponer sobre ese batiburrillo de opiniones bárbaras la concepción sobre lo bueno más racional, la que en Occidente se propugna desde la Ilustración dieciochesca. La respuesta liberal, (...) por el contrario, acepta gustosa la pluralidad de concepciones morales y metafísicas de nuestras sociedades, y lo único que propone es evitar la barbarie haciendo que, en las cuestiones sobre cómo organizar nuestra sociedad, estas diferencias queden de lado y se intente encontrar un mínimo acuerdo entre todos. Por último, la respuesta postmetafísica sugiere aceptar que las cosmovisiones sobre el bien y el mundo no pueden quedar tan totalmente aparte de nuestros modos de organizar la sociedad como quiere el liberal; pero para evitar el conflicto bárbarico entre ellas, persigue hacer plausible sobre las demás una concepción concreta, que, a diferencia de la del ilustrado, aspiraría mucho más modestamente a sólo expandir un escepticismo para con el resto de nuestras propias creencias; autoironía que nos abriría al diálogo con las creencias de los demás y evitaría en ese juego de conversaciones la violencia de la imposición bárbara. BackgroundArtificial intelligence has been described as the “fourth industrial revolution” with transformative and global implications, including in healthcare, public health, and global health. AI approaches hold promise for improving health systems worldwide, as well as individual and population health outcomes. While AI may have potential for advancing health equity within and between countries, we must consider the ethical implications of its deployment in order to mitigate its potential harms, particularly for the most vulnerable. This scoping review addresses the following question: (...) What ethical issues have been identified in relation to AI in the field of health, including from a global health perspective?MethodsEight electronic databases were searched for peer reviewed and grey literature published before April 2018 using the concepts of health, ethics, and AI, and their related terms. Records were independently screened by two reviewers and were included if they reported on AI in relation to health and ethics and were written in the English language. Data was charted on a piloted data charting form, and a descriptive and thematic analysis was performed.ResultsUpon reviewing 12,722 articles, 103 met the predetermined inclusion criteria. The literature was primarily focused on the ethics of AI in health care, particularly on carer robots, diagnostics, and precision medicine, but was largely silent on ethics of AI in public and population health. The literature highlighted a number of common ethical concerns related to privacy, trust, accountability and responsibility, and bias. Largely missing from the literature was the ethics of AI in global health, particularly in the context of low- and middle-income countries.ConclusionsThe ethical issues surrounding AI in the field of health are both vast and complex. While AI holds the potential to improve health and health systems, our analysis suggests that its introduction should be approached with cautious optimism. The dearth of literature on the ethics of AI within LMICs, as well as in public health, also points to a critical need for further research into the ethical implications of AI within both global and public health, to ensure that its development and implementation is ethical for everyone, everywhere. Immanuel Kant's moral philosophy is one of the most distinctive achievements of the European Enlightenment. At its heart lies what Kant called the "strange thing": the free rational human will. This introduction explores the basis of Kant's anti-naturalis, secular, moral vision of the human good. Moving from a sketch of the Kantian will, with all its component parts and attributes, to Kant's canonical arguments for his categorical imperative, it shows why Kant thought his moral law the best summary expression of (...) both his own philosophical work on morality and his readers' deepest shared convictions about the good. This article reviews existing empirical research on the peak-and-end rule. This rule states that people's global evaluations of past affective episodes can be well predicted by the affect experienced during just two moments: the moment of peak affect intensity and the ending. One consequence of the peak-and-end rule is that the duration of affective episodes is largely neglected. Evidence supporting the peak-and-end rule is robust, but qualified. New directions for future work in this emerging area of study are outlined. In (...) particular, the personal meanings associated with specific moments and with specific emotions should be assessed. It is hypothesised that moments rich with self-relevant information will dominate people's global evaluations of past affective episodes. This chapter examines whether, as Jeff McMahan argues, we should not integrate what he refers to as the “deep morality” of war into our military and international public policies and laws, because of the possible negative consequences of doing so. On the basis of feminist epistemology, I argue that McMahan is wrong to think that publicizing and legalizing the deep morality of war will have the negative consequences that he claims. Through a comparison with the Women's Suffrage Movement in the (...) United States, I argue that McMahan's argument is epistemically biased, in that it reflects and incorporates stereotypical views of poor people as stupid, thoughtless, and brutish. We would do better to think hard about how we conceptualize combatants, and to recognize in our arguments that the abilities to reason well and to act morally are not restricted to the upper echelons of society. Jackson begins with the premise that those in the medical profession have a basic duty to be worthy of the trust their patients place in them. Yet questions of the ethics of withholding information and consent and covert surveillance in care units persist. One of the central problems afflicting reductionism in the epistemology of testimony is the apparent fact that infants and small children are not cognitively capable of having the inductively based positive reasons required by this view. Since non-reductionism does not impose a requirement of this sort, it is thought to avoid this problem and is therefore taken to have a significant advantage over reductionism. In this paper, however, I argue that if this objection undermines reductionism, then a variant of it (...) similarly undermines non-reductionism. Thus, considerations about the cognitive capacities of infants and small children do not effectively discriminate between these two competing theories of testimonial justification. (shrink)
